To participate in this survey, you need to enter the 4-digit store number and 11-digit access code from your receipt and can choose either English or Spanish as your preferred language. If you had happened to visit any of its outlets and did not purchase anything, still you can share your experience with its corporate office. For this, you need to click the provided link, fill up the respective fields with your name and email address and the management will get back to you with assurance of maximum support.

Very often, Kohl’s appreciates its patrons for spending their valuable time in taking the survey and at the end of it provides either a validation code, redeemable for a free offer, or an entry in sweepstakes.   1. I am simply trying to fill out survey, from visit yesterday. Have to go through so much garbage----FORGET THE WHOLE THING!!

  2. was going to buy my niece a birthday present my sister and I. walked through the door of our kohl's in fort smith Arkansas, I am epileptic, came in with my service dog and a lady that worked there ran up to us I know she was a manager and said in an angry voice you can not have a dog in here there is no dogs allowed in our store. then she said; the only way you can have a dog in here is if it is a service dog then she proceeded to tell me I NEED to show here the tag! everyone in the stopped what they were doing to hear all of this going on. we could barely get a word out of our mouth and when we finally got to speak we told her she WAS a service dog and I showed her,her tag. it was so embarrassing and upsetting, I do not know if it was because I was African American with a service dog, I know I cannot be the only person that has walked through that door with a service dog. She did this in front of people we did not even know. Some of them came and petted her to let her know she was okay and they talked to us and followed us through the store. What you all need to know is this all service dogs are not big, not all service dogs are ugly. they are there to protect and serve their masters so they can get the help they need. also you all need to know this store is setting you up for a lawsuit! everyone may not be as gracious as I am to let you all know that you cannot do and act this way. She have asked me Is she a service dog it is not what you say but how you say it. I also let her know I would be contacting you alls store. Needless to say I left the gift at the desk and I am thinking about cancelling my card with you all, I would hate for you all to get my service dog money. I gave my money to Wal-Mart where this never has happened so thank you kohl's for making Wal-Mart happy!
